Please arrive 15 minutes prior to your appointment and bring the completed new patient forms with you. For your convenience all patient forms can be downloaded and completed before your initial visit to the office.

Please bring a list of any current medications you are taking and any dental insurance information. Parents and/or guardians must accompany all patients under the age of 18.

Your first visit with Dr. Maria Ang is the best time for you to communicate to us any concerns, fears, and expectations which you may have regarding your dental health.
